Black Skirt Tetra Fin Rot
The fish is suffering from a combination of a disease called fin tail rot as can be seen by the ragged fin and cysts which are the bubble like growths on the fish.

I cannot figure out what it is or how to treat it. It spent most of the time either drifting around at the top and a few days before it died it seems careless flopping around lazily and drifting with the current of the filter.
